This nomination form is for Centex-Regional. However, if you feel that the player should be fast tracked onto the national programme please state clearly at the bottom of the form in the message area.

Nominations should be made between November 1st & December 15th each year for girls in Year 8 upwards.

When nominating players please make them aware of the costs, dates, commitments etc. This information can all be found on this web site.

Nominations are provided by lacrosse teachers or coaches who know the player well.
